After the severe winds and rain that blew across Suffolk County on July 4, causing significant local damage, the county, in collaboration with New York State, has developed an online Damage Self-Reporting Tool for residents and businesses to self-report damage to their homes and/or businesses.
The purpose of this form is solely to report damage information that may help the county and the State to develop a request for possible federal assistance programs. The county encourages residents to submit the form even if the damage is minor.
This form is not an application for relief programs.

Officials said Suffolk County and New York State are trying to develop the clearest, most accurate account of the damage that our area and residents suffered on July 4, to present to federal officials to seek financial assistance for residents and business owners.
